      In my organization the teams participate in post-fall huddles. These are done once the resident has been assessed by a nurse and is safely now settled , usually in their bed. The huddle includes PSWs, Nurses, allied team, etc..
      The huddle is seen as an opportunity to discuss the fall, contributing factors, and people are encouraged to engage and brainstorm for any possible additional interventions to help mitigate the falls risk for the future. The huddle is also documented in he clinical record.

      In the organization I work for we have weekly meeting’s on Monday morning’s to review with the team what our week looks like, if client’s have been sent to acute care over the weekend or any other updates that we need to have. We also do quarterly and discuss the client’s hours, needs and update their care plans unless they needed to be updated sooner due to status change. I feel this helps because it keeps everyone in the circle of care aware of any changes, orders, or any other concerns for the client up to date. It also continues to keep the collaboration of care in a continuous manner.
