Webinar: [Bridging the Communication Gap: Health Literacy in Practice]

Synopsis: Developed by the Centre for Collaboration, Motivation and Innovation (CCMI) this webinar provides an essential overview of health literacy concepts and the communication barriers that affect patient understanding. Participants will be introduced to concepts related to health information, plain language, universal precautions and design. Learn the “Teach-Back” method to ensure client understanding to create more collaborative care environments. [Speaker Bio] Donna Mills is a Public Heath Professional with more than 25 years of experience in public health and over 10 years of experience facilitating Self-Management Programs and professional development workshops across Ontario. She is passionate about helping healthcare providers strengthen communication skills that support health literacy, person-centred care, and meaningful patient engagement. Jehanara Chagani is an Advanced Practice Nurse and Certified Hospice Palliative Care Nurse (CHPCN(C)), specializing in community-based hospice palliative care and a Self-Management Program Faculty. With extensive experience supporting patients and families living with life-limiting illnesses, she is passionate about advancing compassionate and equitable care. In her role as an Advanced Practice Nurse, Jehanara provides clinical consultation, education, and mentorship to healthcare providers caring for individuals with complex palliative care needs. Together Donna and Jehanara As Self-Management Faculty, Donna and Jehanara are committed to advancing health literacy, strengthening healthcare providers’ communication skills, and building healthcare systems where patients and families feel heard, respected, and supported throughout their healthcare journey.
